1. Complete a medical questionnaire by clicking HERE or you can request a form to be sent to you by email or phone. Find our Contact Information HERE.

• If our licensed provider reviews your application and deems you eligible for the program you can complete a quick an easy enrollment either ONLINE or In-Person at JLUXE.

2. Enroll into the Program – Once enrolled there will be a MINIMUM three month payment agreement. What does this mean? This means that by signing the consent/enrollment form you choose to enroll for no less than 3 months, if you decide to stop the medication before three months you are still responsible for the $325 a month payment until three month subscription is over.

3. Get labwork completed – After determining your eligibility you will have to get recommended labs done at either Lab Corp or Quest diagnostics before taking the medication. If you have already had labwork done in the last 2 months you can send lab work in to be reviewed. Labwork is the patients responsibility to cover financially.

• Labs typically monitored for your safety

  i. Comprehensive Metabolic panel (CMP)- this looks at your pancreas, liver enzymes and blood glucose level

  ii. Complete Blood Count (CBC)- this looks at your red blood cells

  iii. Thyroid Stimulating Hormone (TSH)- looks at Thyroid functioning iv. Lipid panel- This looks at your total cholesterol, HDL, LDL, and ratio.

4. Once your bloodwork is completed, reviewed and accepted, a prescription will be sent in for Semaglutide. It will be sent to your home with instructions from our partnered Compounding Pharmacy. If you do not feel comfortable injecting yourself, or you have questions about the instructions provided, you can contact Jen for help or to schedule a time to go to JLUXE in-person for a tutorial or injection FREE OF CHARGE.

5. Re-monitoring lab work. In order to optimize and track weight loss results and safety of medications we will re-order and monitor lab work in intervals of your treatment plan and from there determine if treatment should continue or the patient should come off the treatment plan.
